Citrus tree falling leaves

We got a lemon tree last summer from Blomsterlandet. It has been replanted in a pot, and was placed inside our apartment in front of a west looking balcony door that gets lots of sun. Initially we got an infection with some bugs, sprayed with neem oil (several times) and took off the insects manually. In the spring it started losing its leaves. Then, after being watered with fertiliser every time it was watered, it started doing well and sprouted new leaves, but now the new leaves and the older ones are falling off. If we continue like this, I am afraid the tree will not survive. This area of our apartment gets really hot, but when we open the door it gets chillier and I am not sure if the change of temperature is affecting the tree negatively. I also noticed it had something like mold in the soil. I attach some pictures.
Kindly advise us how to proceed and if you think the plant can be saved. Will it do better if we take it out in the balcony, repot it or prune it? Thank you in advance for your answer.

Generellt bör medelhavsväxter som olivträd och citronträd bara omplanteras vart 3-4 år däremellan räcker det att tillföra nya jord ovanifrån.
